Position Summary

Under the supervision of the Bridgeways Program Manager, the Clinician will provide therapeutic Outpatient Clinical Services (OCS) to youth who are juvenile justice involved or at risk of juvenile justice involvement, and who have behavioral health needs with full-scope Medi-Cal beneficiaries, underserved and uninsured, which qualify them for Full Service Partnership services in the Bridgeways program.

The Bridgeways Program Manager is a qualified clinical supervisor, enabling Clinicians to receive supervision hours toward BBS licensure.

Required Qualifications

  • Masters Degree in counseling, social work or related field
  • BBS-registered AMFT, ASW OR APCC
  • Position will work out of Lifeline's San Diego office .
  • Bilingual (English / Spanish) preferred but not required
  • Must have good driving record and be willing to use own, properly insured, reliable vehicle for the job.
  • Must pass required pre-employment clearances, including background checks, drug testing, and TB testing.

Responsibilities

  • Completes a comprehensive assessment battery, consisting of the trauma-informed Behavioral Health Assessment (BHA) and various other assessment tools as appropriate for each client
  • Collaborate with the youth / family to identify ancillary services that may be needed to supplement therapy, including SUD counseling, psychiatric services, mental health nursing services, and Peer Support Specialist services
  • Communicates regularly with the program's Psychiatrist, Mental Health Registered Nurse, SUD Counselor (if applicable) and Peer Support Specialist regarding client treatment
  • Helps clients who do not have a primary care physician (PCP) to establish a medical home, and document PCP information in Cerner
  • Completes a High Risk Assessment in Cerner within 48 hours of being notified that a client has been discharged from hospitalization
  • Provides crisis management consultation and education as needed
  • Fulfills the role of "single staff person" responsible for OCS clients and families
  • Participates in care coordination
  • Conducts individualized strength-based individual, family, and group therapy utilizing evidence-based treatment modalities
  • Provides collateral contacts, Intensive Home Based Services, Intensive Care Coordination, and crisis intervention for clients as needed
  • Contacts and collaborates with Probation and BHS STAT to ensure coordinated care when OCS youth are transitioning into / out of custody
  • Enters client data into appropriate County systems (County MIS and Cerner) within required timelines
  • Meets productivity and caseload standards dictated by the contract - Minimum of 20 hours per week .
  • Participates in multi-disciplinary team meetings to develop appropriate intervention and support the delivery of behavioral health and support services.
  • Participates in individual weekly supervision.
  • Participates in quality assurance review processes and maintain county documentation standards.
  • Utilizes a trauma-informed, strength-based, culturally respectful approach and abides by the legal and ethical standards of the profession.
  • Completes training required by Lifeline, County BHS, the OPOH, and any other relevant entities
  • Other duties as assigned.
  • Completes other related duties as assigned

Schedule / Compensation

  • Full Time - Nonexempt position.
  • Monday- Friday 40hrs / week
  • Hybrid schedule : Minimum of 3 days in the office / in the community
  • $30.00- $33.00 / Hour
  • Differential pay offered for bilingual skills. EEO.

Additional Requirement

  • Must have a valid driver's license and reliable transportation with a good driving record.
  • Willing to use own, properly insured, reliable vehicle for the job (mileage reimbursed).
  • Employment subject to clearances, including background finger printing checks, driving record, tuberculosis testing, and drug testing.

Physical Demands / Work Environment

This role involves a dynamic range of activities, requiring physical agility and mental resilience. Employees must be capable of :

  • Mobility : Frequent standing, walking, sitting, and the ability to navigate stairs.
  • Dexterity : Regular use of hands for manipulation of objects, tools, and controls.
  • Physical Actions : Ability to stoop, kneel, crouch, crawl, and maintain balance.
  • Sensory Tasks : Engaging in conversations, listening, and requiring precise vision abilities such as close, distance, color, and peripheral vision, along with depth perception and focus adjustment.
  • Lifting : Occasionally required to lift and / or move items up to 15 pounds.
  • Stressful Conditions : Preparedness to work in highly stressful situations, addressing significant mental and emotional demands.
  • Travel and Exposure : Necessity to travel to client homes, potentially encountering varied weather conditions.
  • Equipment Operation : Frequent use of office equipment, demanding repetitive hand movements and fine coordination, including computer use.
