The Community Health and Outreach Manager manages the Community Health and Outreach program that promotes wellness and prevents disease in the community by engaging Sedgwick County residents and community partners.
The position oversees the Community Health Assessment and Community Health Improvement Plan, and chronic and infectious disease health education and prevention.
The position supervises a Community Health Worker that performs outreach and connects clients to services. Employee will assume a command level leadership position during public health emergencies.
Oversee community health initiatives
- Develop education and outreach work with community partners and evidence-based curricula
- Regularly evaluate and update Community Health and Outreach Program policies and procedures based on changes with governing agencies and changes to industry standards and research
- Develop surveys and other evaluation processes to identify gaps in services and evaluate eligibility to meet community needs
- Identify and track performance metrics using input from national and state agencies and organizations, policies, and best practice
- Research and coordinate grant opportunities to assist programs as needed within the Sedgwick County Health Department.
Manage Community Health and Outreach Program
- Manage Community Health and Outreach program development, agreements, contracts, grants and budgeting and provide guidance on program expenditures
- Review interview and hiring materials and coordinate the full life cycle recruitment process for Program staff
Establish and Develop Community Relationships
- Develop and maintain relationships with external partners, ensuring program requirements / services are updated
- Evaluate impact from outreach events and strategically plan future events for the next year
- Lead and assist in planning and implementing studies and collecting and analyzing public health data
Minimum Qualifications : Bachelor’s degree in public health or a biological, medical, or health science or related public service field.
Two years of work experience in a leadership role in public health or related field, including all of the following (which can be accomplished at the same time) : one year of work experience in planning and developing programs, one year of work experience working with community partners, one year of experience or training in data management and / or data analysis, and one year of supervisory experience and / or experience evaluating staff performance.
OR - Six years of work experience in public health or a biological, medical, or health science or related public service field.
Two of the six years must be in a leadership role in public health or related field that includes all of the following (which can be accomplished at the same time) : one year of work experience in planning and developing programs, one year of work experience working with community partners, one year of experience or training in data management and / or data analysis, and one year of supervisory experience and / or experience evaluating staff performance.
Per Sedgwick County Policy, this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance.
Must complete Federal Emergency Management Administration (FEMA) IS 100, 200, 700, and 800 courses within 30 days of employment.
Preferred Qualifications : Three years of experience in coalition building, facilitation and / or research. One year of technical writing experience.
Three years of work experience supervising and evaluating employees. Two years of health education experience. Master’s degree in public health or a biological, medical, or health science or related public service field.
Certified Health Education Specialist (CHES), Certified in Public Health (CPH)
