Essential Job Duties / Responsibilities :
- Provide direct patient care and assessments in ambulatory health care centers and other settings
- Conduct geriatric assessments, medical history, physical exams, diagnosis, and treatment
- Participate in patient care rounds and conferences to communicate patient management strategies
- Collaborate with the multidisciplinary team to ensure successful patient care
- Advocate for patients and provide education to patients and families
- Manage resource utilization and reimbursement for services
- Facilitate discharge to appropriate level of care and use preferred providers for additional services

Education and Experience Criteria :
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Board Eligibility is required; Board Certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
- Current, active MD licensure in state of employment is required
- Minimum of 2 years' clinical experience in geriatric, adult, or family practice setting required
- Current DEA number for controlled substances required
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days of employment
