Join Our Dynamic Team as a Hospice Care Coordinator at Geiss Med Hospice for the Inland Empire region!

What You'll Do :

  • Build Lasting Relationships : Develop and nurture strong connections with referral sources such as physicians, hospitals, long-term care facilities, and assisted living facilities.
  • Engage and Educate : Deliver compelling presentations on our in-service offerings to various referral sources and continuously follow up to address their needs.
  • Be the Expert : Educate healthcare professionals and community members on the comprehensive benefits of hospice care services.
  • Liaise and Coordinate : Act as the essential link between Geiss Med Hospice and referral sources, ensuring that patient needs are met, and expectations are exceeded.
  • Resolve and Innovate : Address any concerns or issues faced by referral sources promptly and effectively and provide innovative suggestions to improve our marketing strategies.
  • Community Ambassador : Represent Geiss Med Hospice at community events to promote a positive image and awareness of our services.
  • Seamless Transitions : Work closely with discharge planners to ensure smooth patient transitions from hospitals and nursing facilities to their homes.

What You Bring :

  • A dynamic personality with excellent communication and presentation skills.
  • A passion for healthcare and a commitment to improving patient care.
  • Strong organizational skills and the ability to manage multiple relationships.
  • Experience in a healthcare setting, with a solid understanding of hospice care services.
  • Two years of experience in coordinating business development and providing education to patients, insurance companies, physicians, hospitals, nurses, community resources, other healthcare specialists, and the local community.
  • The ability to work collaboratively within an interdisciplinary team and independently.
