Position Summary : Under the Agency’s policies and professional requirements, the Re-Entry Team Lead provides direct care services : intakes, assessments, and coordinates linkage into needed service.

They coordinate Targeted Investments Program (TIP) and BTG Reentry client referrals, case management services as needed, tasks involved in serving the clients of the TIP service.

They shall fully comply with all TIP requirements per AHCCS guidelines. Works with other Program staff, Program Manager and Sr.

Director, and provides health and wellness and recovery-oriented services. Participates in program evaluation procedures and professional record keeping.

This position works under the direct supervision of the Program Supervisor; however, the BTG Re-Entry Team Lead also provides services to TIP patients.

Coordinates the handling of new referrals; includes contacting the referral source immediately upon receiving the referral.

Reviews all incoming referrals to ensure appropriateness of referral and coordinates and discusses any relevant concerns with referral source.

Assigns the referral to the appropriate team member and works with the team member to ensure follow up is complete.

Assist the BTG Reentry Supervisor with networking activities that aim to educate the community and probation department about TIP / BTG Reentry services and generating more referrals.

Provides direct face to face community-based services as needed to assist TIP / BTG Reentry patients with engagement into necessary services.

Provides outreach for the purpose of engaging patients into TIP / BTG Reentry services. Orients potential patients about TIP / BTG Reentry program.

Ensures ongoing collaboration, including communication of appropriate clinical information with other involved parties as appropriate.

Coordinates care with care team, patient’s family, behavioral and general medical providers, community resources, and other involved support as indicated.

Assists patients with connection to physical health services, including Primary Care services and medical appointment follow up.

Provides direct care wellness management and recovery strategies using strength-based engagement. Ensures linkage into community services and resources.

Actively engages patients into services by utilizing motivational interviewing techniques creating an alliance that will support their success in reaching their treatment plan goals.

Employs therapeutic interventions for life skill development, participating in meaningful activities and daily living skills appropriate for identified goals on treatment plan including assertive outreach and engagement strategies.

  • Assume the day-to-day responsibilities of the BTG Reentry Supervisor during absence of program supervisor; perform as the contact person of the TIP / BTG Reentry team during absence of the BTG Reentry Supervisor.
  • Produces an average of 50% productivity for hours worked.
  • Provides excellent customer service to patients, internal and external provider agencies, state entities, and other stakeholders (e.

g., probation officers, advocates, family members, residential staff, hospital staff and vocational rehabilitation, etc.) as needed.

Qualifications

High School Diploma or GED, as well as a combination of behavioral health education and work experience totaling a minimum of three years;

or an Associate’s degree in any field, plus two years of work experience in a behavioral health service.

  • Customer service skills, especially good communication skills are needed
  • Ability to work independently, with flexibility, and within prescribed deadlines
  • Basic computer skills
  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
  • Must have a valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience and meet requirements of Terros Health’s driving policy.

Driving is an essential function of this position.

Must pass a TB test.
