Job Summary :
We are seeking a dedicated and experienced Case Manager to join our team. The ideal candidate will provide comprehensive case management services, facilitating communication and coordination among healthcare team members to ensure quality and cost-effective outcomes for our members.
This role involves intensive case management, including assessment, planning, implementation, coordination, monitoring, and evaluation of members' needs.

Salary : $43 - $65 DOE
Contract Length : 6+ months
Key Responsibilities :
Care Management (85%) :
Conduct comprehensive health needs assessments and clinical assessments.
Develop and implement individualized care plans with SMART goals.
Perform post-discharge assessments to identify member needs.
Coordinate care and services with members, their families, and healthcare providers.
Facilitate referrals to behavioral health services and community resources.
Participate in interdisciplinary team meetings and collaborate on case resolutions.
Advocate for necessary funding, treatment alternatives, and coordination of care.
Administrative Support (10%) :
Document all case interventions following organizational protocols.
Maintain appropriate documentation of patient care and progress.
Other Duties (5%) :
Participate in departmental responsibilities and support short- and long-term goals.
Complete additional projects as assigned.
Required Qualifications :
Associate degree in nursing (ADN) or related field, with 3 years of clinical or managed care experience, or an equivalent combination of education and experience.
Current, unrestricted Registered Nurse (RN) license to practice in the state of California.
Preferred Qualifications :
Bachelor of Science in Nursing (BSN) or related field.
Case Management Certification (CCM).
Bilingual in English and one of CalOptima Health’s defined threshold languages (Arabic, Chinese, Farsi, Korean, Spanish, Vietnamese).
Knowledge & Abilities :
Establish and maintain effective relationships with internal and external contacts.
Work independently with sound judgment.
Communicate clearly, both orally and in writing.
Manage multiple projects and work in a fast-paced environment.
Utilize computer software (Microsoft Office : Word, Outlook, Excel, PowerPoint).
Physical Requirements :
Ability to read information from computer screens and printed materials.
Clear verbal communication for telephone and face-to-face interactions.
Manual dexterity for typing, writing, and prolonged sitting.
Ability to lift and move objects up to 25 pounds.
Work Environment :
Work is typically indoors and sedentary, with variable work hours and travel as needed.
Moderate noise level.
