JOB DESCRIPTION
TITLE : LPN Charge Nurse
REPORTS TO : NURSING SUPERVISOR AND DIRECTOR OF NURSING
JOB SUMMARY :
Provides direct nursing services to residents including but not limited to observation, recording and monitoring, treatments and therapy, and medication administration.
ESSENTIAL JOB FUNCTION AND RESPONSIBILITIES :
1 Effectively plans and implements resident care according to physician orders, current policy and procedure, and standards of care.
Makes daily resident visits to observe and evaluate the physical and emotional status of each resident.
Administers medications as ordered by physician compliant with facility standards and Local, State, and Federal regulations.
Renders direct nursing care to facility residents.
Sets up treatment trays; prepares, instruments and other equipment and assists physicians(s) as necessary.
Regularly reviews care plans for residents to ensure proper delivery of care.
Administers treatments as ordered by physician.
Administers emergency aide as required.
Actively participates in the CQI programs for the Department of Nursing appropriate to the scope of resident care delivery.
Effectively participates in Resident Care Conferences through the writing and maintaining of the nursing portion of care plans and identifies concerns and needs.
Develops goals and approaches in the care plan of residents.
Monitors and records resident care data consistent with existing policies and nursing practice standards.
Takes and records vital statistics as ordered.
Transcribes and notes physician orders to appropriate resident forms.
Maintains proper documentation of resident records and all other reports according to the established policies and procedures of the facility and consistent with Local, State, and Federal regulations.
Reports to supervisor, physician, and families any significant changes in the resident’s condition. Documents accordingly.
Effectively supervises and administers the delivery of resident care in the resident care area assigned.
Assists Supervisor with supervising and directing the work of the nursing assistants on each unit.
Performs employee performance evaluations and makes appropriate recommendations for personnel related action such as promotions, transfers, or reassignments.
Assigns and delegates the work of all staff assigned to the unit.
Takes disciplinary or corrective actions in conjunction with the proper supervisor and administrative personnel against employees in violation of facility policies and / or procedures.
Provides guidance and on-going training to co-workers as needed.
Maintains emergency equipment.
Attends department meetings and inservice programs as required or per facility policy.
Uses initiative and judgment in problem solving abilities and regarding residents, families and staff.
Observes all fire safety and related rules and regulations.
Takes any steps required to ensure a safe and secure environment for residents, families and staff.
Maintains confidentiality in all issues relating to or involving residents and staff.
Exhibits teamwork mindset in the daily activities of the job.
Promotes a sense of unity and teamwork in own work area and throughout the facility.
Is responsible team member who is honest and accountable for own actions.
Supports the member of their team.

Qualifications
EDUCATION / MINIMUM REQUIREMENTS :
Graduate from an accredited school of nursing.
STATE / FEDERAL MANDATED LICENSURE OR CERTIFICATION REQUIREMENTS :
Current State of Connecticut license as a Licensed Practical Nurse.
INGRAHAM MANOR MANDATED EDUCATIONAL REQUIREMENTS :
General orientation at time of hire. Fire / Safety / Infection Control annually. Other programs as mandated by Ingraham Manor.
SPECIAL REQUIREMENTS :
Must demonstrate patience and kindness toward residents and families. Must possess good observation and communication skills.
Indicates a nonessential function, for which an otherwise qualified candidate could seek reasonable accommodation and thereby successfully discharge the duties of this position.
PHYSICAL REQUIREMENTS :
Must have the ability to perform all aspects of nursing care. Must be able to lift a minimum of 50lbs. Must be able to endure a prolonged period of extensive walking, standing, stooping, pushing, and pulling.
Moderate periods of sitting, twisting, reaching, occasional crouching, crawling, kneeling, and balancing. Vision, hearing, smell and tactile sensation corrected or accommodated to normal.
WORK ENVIORNMENT :
Regular contact with residents, body substances, and hazardous chemicals, 75 degree temperature, varied lighting, regular contact with soap and water, moderate noise level, use of various types of lifting devices.
COGNITIVE REQUIREMENTS :
Ability to read, write, and follow written and oral instructions.
