Job Details
- Required : At least 2 years of care management with discharge planning experience in a hospital setting
- Required : CarePort (WellSky) and EPIC experience
- Our Case Managers are service based, they follow provider teams. Coverage would be for any of our adult teams.
- Case Management Assistants help with CarePort referrals.
- Ratios : 1 : 28.
- Our RN CMs are service based and we utilize our full-time floats in these assignments as needed when there are vacations, call-offs, PTO, and other scheduled days off.
