Palliative Care Registered Nurse

We are looking for a compassionate and skilled Palliative Care Registered Nurse to join us in supporting patients and families.
Responsibilities :
- Provide preventive and supportive care to palliative patients and families, utilizing assessment, intervention, and teaching skills to maximize patient comfort and enhance quality of life
- Collaborate with the patient / family, attending physician, and other members of the interdisciplinary team in providing patient and family care
- Document the problems, appropriate goals, interventions, and patient / family response to palliative care
- Effectively communicate with members of the team, clinical supervisor, and the patient's PCP regarding changes in patient status and / or patient needs
- Maintain accurate and relevant clinical notes regarding the patient's condition
Requirements :
- Associate's / Bachelor's degree in Nursing
- 2 years of prior skilled Registered Nurse experience
- Current RN license to practice in the state
- CPR Certification
- Competence in computer programs Microsoft Outlook, Word, and Excel
- Prior clinical experience and strong compassionate phone skills
