Provide care for our community of recovering patients, ushering them back to a life of independence.
Create and follow personalized plan of care in collaboration with RN and the interdisciplinary team.
Educate our patients and their families on next steps in continuing care and improvement of day to day life including : therapy, excercise, and treatments.
Coordinate and perform medical treatments to enhance the well-being of the patient with the goal of enhancing patient health.
