What does success look like as a Psychiatric Nurse Practitioner?
- Comprehensive Psychiatric Assessments : Conduct in-person or telehealth assessments and collaborate with the IDT to develop personalized care plans.
- Collaboration with Mental Health Counselors : Partner with the Mental Health Counselor to guide IDT meetings, bridging knowledge gaps on behavioral health issues, and empowering the team with tools for effective interventions.
- Behavioral Health Education : Educate IDT members on mental health disorders, substance abuse, dementia-related behaviors, and goal setting for meaningful behavioral interventions.
- Integrating Behavioral Health Care : Work closely with clinicians to integrate behavioral health into participants' care plans seamlessly.
- Building Community Partnerships : Establish and maintain strong communication with community behavioral health specialists, ensuring collaborative care.
- Coordination with Geriatric Psychiatrists : Manage schedules and facilitate the exchange of critical information to make the most of geriatric psychiatric consultations.
- Inpatient Follow-Up : Support participants admitted to psychiatric facilities to ensure issues are addressed, and follow-up care is well-planned.
- Behavioral Health Diagnosis : Evaluate participants with complex behavioral health issues to develop accurate diagnoses and tailored care plans.
- Team Training & Support : Provide behavioral health education to team members, equipping them with knowledge to enhance participant care.
- Contribute to Organizational Growth : Embrace additional opportunities and responsibilities as needed, actively participating in the continued growth and success of the organization.
What does an ideal candidate look like?
- Passion for serving high-risk seniors and frail older adults.
- 3+ years of experience providing behavioral healthcare to elderly or complex populations.
- Experience working with Interdisciplinary Teams in a healthcare setting.
- Active Psychiatric Mental Health Nurse Practitioner State License without restrictions.
- State CDS Certification and DEA License.
- NPI License required.
- PACE experience preferred but not required.
- CPR and First Aid certification (or ability to obtain within 60 days).
- Valid CA driver's license, personal transportation, good driving record, and auto insurance (as required by state law).
$130,000 - $180,000 a year
