- Established and expanding organization adding physician to their program
- Monday to Friday daytime hours with light call schedules shared among the team
- PACE Program with geriatric patients from age 55 and older
- See patients to increase quality care or post-acute to decrease chance of readmissions
- Work with NPs and others on the team for overall patient care
