Summary
SUMMARY : The AHD Clinical Pharmacist, under the direction of the Directory of Pharmacy, assists in the planning organizing and monitoring of the delivery of pharmacy.
Performs related duties as required.
DUTIES & ESSENTIAL JOB FUNCTIONS : NOTE : The following are the duties performed by employees in this classification, however, employees may perform other related duties at an equivalent level.
Not all duties listed are necessarily performed by each individual in the classification.
1. Assists the Director of Pharmacy in developing and maintaining the drug formulary.
2. Coordinates education activities for medical, nursing, and pharmacy staff; orients new employees on pharmacy practices and procedures.
3. Coordinates the use and monitoring of expensive medications with the Pharmacy Director; collects and reports adverse drug reactions to Pharmacy and Therapeutics.
4. Keeps informed of new drug and innovations in the pharmaceutical field, maintains awareness of current literature and information for medical staff, serves as a resource for the professional staff and answers technical pharmaceutical questions.
5. Monitors prescribing habits of prescribers for long range planning and cost analysis. Performs literature searches for medical staff enabling dissemination of the most current data on treatment modalities.
6. Represents Pharmacy Services on various committee, staff meetings and professional meetings as needed.
7. Updates the medication monitoring guidelines; performs medication monitoring and reports findings to the Director of Pharmacy;
prepares drug monographs for presentation to committees, staff, physicians and other staff.
8. Works with the Director of Pharmacy to coordinate and administer pharmacy services with AHS. MINIMUM QUALIFICATION :
Education : Graduation from an accredited school of Pharmacy.
Education preferred : Doctor of Pharmacy (PharmD).
Minimum experience : One year pharmacist experience preferred.
Required Licenses / Certifications : Valid Pharmacist License issued by the California State Board of Pharmacy.
Additional Information
Ability to complete clinical projects and work with various departments to education and launch new drugs. Ability to go on rounds and attend Codes.
Must have inpatient Pharmacist experience.
