Physician Assistant Community Based

Geisinger Care at Home team is seeking an experienced Physician Assistant (APP) for a community-based position, covering Luzerne and Lackawanna counties

Job Duties

The Geisinger Care at Home Advanced Practice Provider is a field-based patient direct care position performing health risk assessments, depression screening and addressing care gaps for Geisinger Health Plan Gold members.

ADDITIONAL INFORMATION :

Work is typically performed in the patient’s home.

Approximately 5-10% of the visits are virtual telemedicine.

APP must reside in geographic area due to nature of position.

Schedule can be 5 8-hour days or 4 10-hour days.

There is no call, weekend, or holiday work.

Advanced Practitioners receive the following benefits :

EDUCATION REQUIREMENTS :

A minimum of two years of Advanced Practice Provider experience is required.
