The On Call, Mental Health Associate provides mental health services and behavioral interventions for medically compromised adults in an outpatient rehab setting.
Coordinates program schedules.
Location : This position is based out of LifeWorks NW's Sapphire at Cedar Crossing, Portland
Pay : $23.45 starting hourly pay.
Essential Responsibilities :
- Serves as resource for other staff members regarding administrative questions or behavioral interventions. This person has no administrative supervisory role (they do not do performance evaluations, hire or do disciplinary actions) they may have input on the evaluation which is done by the Program Coordinator / Director.
- Provides behavioral interventions, case management and skill training to client and Adult Foster Home (AFH) staff as defined in the treatment plan.
Encourages clients to express their feelings and discuss what is happening in their lives, and help them to develop insight into themselves and their relationships.
Guides clients in the development of skills and strategies for dealing with their problems.
- Counsels clients and patients, individually and in group sessions, to maximize the quality of their lives. Collects information about clients through interviews and observation.
- Develops and presents group activities or therapies that are appropriate for treatment such as art groups, community outings, movement, reminiscence, etc.
Collaborate with other staff members to perform clinical assessments and develop treatment plans.
- Assists clients with activities of daily living including eating, bathing, dressing, toileting and grooming, alone or with assistance;
- observing patients' conditions, measuring and recording food and liquid intake / output, vital signs and reporting changes to professional staff;
feeding clients who are unable to feed themselves; providing help walking and exercising.
Coordinates services with other LifeWorks NW staff and with external organizations such as government agencies, social services providers, and healthcare providers.
Act as client advocates to coordinate with Skilled Nursing Facility (SNF) required services or to resolve emergency problems in crisis situations.
- Provides crisis intervention to clients experiencing difficulties.
- Documents and maintains client records in electronic health record system within 24 hours of client care to ensure compliance with LifeWorks NW and contractual guidelines.
Maintains confidentiality of records relating to clients' treatment.
- Transports clients in LifeWorks vehicle ensuring their safe transportation.
- Some evenings and occasional weekends required.
Qualifications :
- A bachelor's degree in behavioral sciences field or at least three years relevant work, education, training, or experience.
- This position requires a valid and active QMHA credential certification through MHACBO upon hire.
- Typically the more senior / experienced person on a team.
- Demonstrated ability to communicate effectively; understand mental health assessment, treatment and service terminology and individual, group, family and other counseling techniques;
program policies and procedures for services and supports identified in an ISSP. (Individual Services and Supports Plan.)
- Prior experience working in skilled nursing facilities or leading behavioral groups preferred.
- Requires valid driver's license and verification of safe driving record from DMV and the ability to pass Ride Connection driving test.
- Requires ability to work some evenings and / or weekends.
