Responsibilities :
- Deliver exceptional nursing care to patients in the comfort of their homes, ensuring their safety, comfort, and overall well-being.
- Administer medications and treatments as prescribed by healthcare professionals, adhering to the highest standards of accuracy and patient safety.
- Monitor patients' vital signs, observe any changes or concerns, and communicate effectively with the healthcare team.
- Collaborate with physicians, therapists, and other healthcare providers to develop individualized care plans that meet the unique needs of each patient.
- Educate patients and their families on healthcare procedures, medication management, and strategies for disease prevention and self-care.
- Document patient progress, observations, and interventions promptly and accurately, maintaining meticulous records.
- Uphold infection control protocols and maintain a clean, organized, and welcoming environment.
- Provide emotional support, companionship, and encouragement to patients and their families during their healthcare journey.
Requirements :
- Current and valid LPN license in the state of Florida.
- Minimum of 1 year of clinical experience as an LPN, preferably in a home health setting.
- Excellent communication, interpersonal, and critical thinking skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Possession of a valid driver's license and reliable transportation.
