As a Hospice HealthLicensed Vocational Nurse, here’s a big-picture view of what you’lldo :

• Provide home-based care to patientswith a prognosis of six months or less to live, focused on qualityof life
• Work as part of an interdisciplinaryteam comprised of a physician, RN, CNA, HHA, social worker,chaplain and volunteers to carry out the patient’s plan ofcare
• Perform nursing services which areplanned, delegated, and supervised by the registerednurse
• Assess and provide patient andfamily / caregiver education and information pertinent to diagnosisand plan of care
• Take a holistic approach to carethat engages the patient’s mind, body, spirit andfamily

A few must-haves for HomeHealth Licensed VocationalNurses :

• Graduate of anaccredited vocational nursing program and active LVN license inCalifornia
• One year ofhospice LVNexperience
• CPRcertification
• Valid Californiadriver's license, auto insurance, and reliabletransportation
• Knowledge ofstate and federal home health and hospiceregulations
• Clinicalproficiency in nursing, strong communication skills andcompassion
