Responsibilities :
- Provide comprehensive nursing care to patients in a hospice setting
- Assess and monitor patients' physical and emotional well-being
- Develop and implement individualized care plans based on patient needs
- Administer medications and treatments as prescribed
- Collaborate with interdisciplinary teams to ensure holistic patient care
- Provide education and support to patients and their families
- Maintain accurate and up-to-date patient documentation
Qualifications :
- Valid and current Registered Nurse (RN) license
- Minimum of 6 months of experience in hospice
- Proficiency in using electronic medical record systems, with experience in HCHB preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Compassionate and empathetic attitude towards patients and their families
- Valid driver's license and reliable transportation
