Job Summary : The Case Manager will assist in providing intensive treatment and intervention services. Case Manager will ensure that patients services needs are met.
As well as ensuring that cases are staffed and are addressing and advocating on behalf of their patients.
Supervisory Responsibilities :

Duties / Responsibilities :
- Assists in assessment / linkage and referral of individuals in crisis to appropriate
- Assists in providing case management during the individuals (assessments, referrals, linkages, advocacy, transportation, documentation, and monitoring).
- Assists and works with community case managers
- Arranges and provides transportation for other services as needed
- Provides crisis intervention services as needed
- Assists and participates on call responsibility
- Works closely with clinical team
- Maintains digital chart and electronic chart for compliance
- Ensure that staffing needs are met
Required Skills / Abilities :
- Excellent knowledge of case management and crisis intervention services.
- Proficient in MS Office and computer systems (e.g. patient management software)
- Excellent communication and interpersonal skills
- Excellent organizational skills
- Strong analytical and problem-solving skills
- Ability to prioritize task
- Excellent time management skills with a proven ability to meet deadlines
- Performs other duties as required
Education and Experience :
Bachelor’s degree and one year's experience working with the mentally ill, those with intellectual disabilities, and / or substance abuse.
Physical Requirements :
- Prolonged periods of sitting at a desk and working on a computer.
- Must be able to lift to 15 pounds at times.
Ability to commute :
Havertown, PA , Philadelphia PA, Delco PA, Montco PA, Bucks County and surrounding Areas. This position is not a remote position.
