RN-Pediatric Case Management Supervisor

Full Time

Monday-Friday 9am-5pm

As the Pediatric RN Case Management Supervisor, you will be responsible for coordinating pediatric patient care services.

Reporting to the Director of Patient Services and working collaboratively with the RN Case Manager and interdisciplinary team members, you will ensure services are provided and coordinated according to regulatory requirements.

Responsibilities Include :

  • Coordinates patient's care that is cost-efficient and reimbursable under the patient's insurance plan and approved by physician
  • Reviews insurance plan information and verifies eligibility with operations department.
  • Reviews patients chart to determine all services are provided as per plan of care
  • Understands and interprets the CHHA reimbursement policy and determines the billing source for patient care.
  • Reviews clinical documentation to ensure care coordination and compliance with plan of care
  • Participates in QAPI activities and Quality Improvement chart audits and other activities to ensure quality of care provided by clinical staff.
  • Participates in staff development programs including clinical orientation, supervisory field visits, and in-service of clinical staff.
  • Reviews documentation to ensure documentation accurately reflects clinical status of the patient and skilled need for CHHA services.
  • Reviews clinical documentation to ensure compliance with discipline frequencies and duration of services on 485(physician orders)
  • Participates in care coordination meetings to ensure adherence to frequency and duration of services as per physician's order.

Reviews patient progress toward goals, continued need for services and discharge planning.

  • Communicates frequently with RN Case Manager and interdisciplinary team members to ensure services are provided and revises the goals and interventions to patient care, as needed.
  • Acts as a coordinator of the multidisciplinary team providing care to patients
  • Creates / updates plan of care with input from appropriate interdisciplinary health team members based on goals mutually acceptable to the patient and family.
  • Documents coordination of care in medical record as needed
  • Supervision of pediatric nurses including joint visits and annual evaluations as per Department of Health guidelines
  • Monitors the effect of the care plans on the patient health status and informs physician and other personnel of changes in the client's needs.
  • Initiates and maintains written and verbal communications to assure patient optimal patient care
  • Plans, facilitates, and documents patient's discharge from the program.
  • Completes all other required documentation in a timely manner.
  • Participates in the orientation of pediatric nursing staff.
  • Rotates responsibility for weekend and holiday 24 hour on-call coverage with other nurses.
  • Performs other related duties and projects, as assigned, including home care visits, orientating new nurses, community outreach and liaison with hospitals and other providers.

The ideal candidate will :

  • Have license and current registration to practice as a Registered Professional Nurse in New York State.
  • BSN Degree from an accredited school of nursing
  • BLS required
  • 3+ years of prior experience working in home care / CHHA environment
  • Supervisory experience a plus
  • Prior experience working with the pediatric population a plus
  • Full vaccination for COVID-19 required
  • Access to reliable transportation or able to access public transportation reliably and able to transportation within Brooklyn, Queens and Manhattan, depending on case assignment
  • Ability to work holidays and weekends on a rotating schedule
