As a Registered Nurse Case Manager, you will provide direct care to patients in their homes or care facilities. Your role goes beyond offering medical assistance;

it involves attentively listening to patients' needs and respecting their wishes. Building trusting relationships with patients and their families is a cornerstone of this position, allowing you to serve as the primary communication link within our interdisciplinary team of holistic care providers.

Registered Nurse Case Manager Responsibilities :

Provide direct patient care, including assessing and monitoring patients' conditions, administering medications, and performing treatments

Collaborate with healthcare team members to develop and implement individualized care plans for patients

Coordinate and facilitate communication between patients, families, and healthcare providers

Conduct ongoing assessments to evaluate the effectiveness of the care plan and make necessary adjustments

Educate patients and their families on disease management, treatment options, and self-care techniques

Utilize case management principles to ensure continuity of care and appropriate resource utilization

Registered Nurse Case Manager Benefits & Schedule :

Monday to Friday 8 : 30 AM to 5 : 00 PM

No Mandatory on-call required
