Job Description
KPG Home Health seeks a skilled and compassionate Home Health Registered Nurse to join our dynamic team. As a Home Health RN, you will be crucial in providing quality healthcare visits to patients in the comfort of their homes throughout Central Phoenix and Mesa.
Responsibilities :
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed by physicians.
- Monitor and document patient progress, ensuring accurate and timely reporting.
- Educate patients and their families on proper healthcare procedures and disease management.
- Collaborate with physicians and other healthcare professionals to coordinate comprehensive patient care
- Maintain accurate and up-to-date patient records.
Requirements :
- Current RN license in the state of Arizona.
- Minimum of 4 +months of RN experience in home health
- BLS certification.
- Strong clinical and assessment skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and as part of a collaborative team.
- Reliable transportation and willingness to travel to patient homes.
- Oasis Start of Care experience required
This role offers :
- Mileage Reimbursement
- $83 - $100 per visit for PRN
- $1,250 - $1,440 per week for travelers
- 24 / 7 clinical support
- Company-issued tablet
Position
- Visits per week : 10- 18
- Adult Patients
- Mon- Fri, No weekends
- SOCs, Supervisory visits, and Routine visits
