About the Job
Waypoint's Family Preservation Team seeks compassionate and adaptable In-Home Clinical Case Managers to join our powerful mission of fostering permanency for families in the greater Manchester, NH area.
As a Permanency Therapist, you will provide clinical interventions and case management to children and families through a trauma-informed, strength-based approach.
Working across multiple systems like homes, schools, child protection, and courts, you will conduct family and individual clinical work with adults, children, foster families, and birth families.
Utilizing solution-focused casework, you will build consensus on the needs of children and families, develop treatment plans to strengthen families, and help them achieve safety, permanency, and well-being.
Referred by child welfare agencies, your caseload will be high-need families requiring consistent face-to-face support.

Minimum Requirements :
- Master’s Degree with a major in social work, counseling, marriage & family therapy and at least 2 years direct work experience in assisting children and families or
- Bachelor’s degree with a major in social work, counseling, marriage & family therapy and at least 5 years direct work experience in assisting children and families
- Ability to train in or experience in evidence-based practices desired
- Prior work in home visiting along with understanding of DCYF.
- Support Program and Team with afterhours, On-Call Services for clients and families.
- Computer Skills and Experience with Microsoft Outlook, Word, Excel is required. Previous experience with Office 365 and SharePoint desired.
- Following confidentiality policies regarding client sensitive information, Waypoint Technology Policies and adhering to Waypoint’s Hybrid Model.
- Successful criminal check, No history of abuse, neglect, or exploitation.
- Must have valid driver’s license, good driving record, reliable transportation, and liability insurance coverage.
Abilities Needed :
- Ability to respect clients from a variety of backgrounds and situations.
- Ability to keep good records and write treatment plans.
- Ability to manage complex cases.
- Ability to travel within the state to provide home visits with possible transportation of clients and / or family to and from.
- Ability to collaborate with peers, Leadership and the Team for Program changes or case related client care.
- Be flexible with schedule to meet family's needs.
