The Registered Nurse will utilize the Nursing process to plan and implement patient care to achieve optimal patient outcomes.
She / he will educate patients and caregivers to achieve self-management of health and / or chronic disease. The RN will assess patient response to care plan and revises as needed.
She / he will coordinate aspects of care plan requiring testing and / or referral to specialty care. Identifies and addresses barriers to care.
Candidate may be required to cross cover across Richmond Health Network.
Requirements :
- BSN preferred.
- Graduate of an accredited School of Nursing.
- Current registration in New York State as an RN, Diploma and Licensure required.
- BLS certification required.
Salary Range : $90,000 - $100,000
