As part of our innovative homecare team you will apply the nursing process integrating principles that reflect an inclusive approach focused on best practice and nursing excellence pillars of our Magnet philosophy.

Daily collaboration with an interdisciplinary team including providers underscores our commitment to combine expertise across the continuum of care to achieve the best outcomes for our patients.

Provides acute psychotherapeutic and / or supportive interventions for patients and caregivers with Behavioral Health needs.

Practices professional nursing as a registered nurse within the legal and ethical framework established by the Connecticut Nurse Practice Act and the American Nurses Association Scope and Standards for Nursing Practice and Code of Ethics, American Nurses Association-Home Health, American Nurses Association-Hospice and applicable regulatory standards.

Oversees the plan of care for approximately 25-30 patients within Middlesex County (not the entire state) with acute care needs including but not limited to wound, drains, chronic disease management etc.

This nurse will coordinate care alongside an interdisciplinary team under physician orders, to ensure best outcome for patients.

Each nurse has condensed territory that he / she will cover and sees 6 visits per day or the equivalent of that, with adjustments based on travel if appropriate.

Requirements include working 1 weekend per month and 2 holidays per year only.

Minimum Qualifications :

Graduate of an accredited School of Nursing

Current and unrestricted license in good standing to practice as a Registered Nurse in the State of Connecticut or the ability to obtain.

Possess the knowledge and skills necessary to assess health needs of patients and to perform nursing care in a safe, caring manner
