The Bridge Family Center is looking for a Case Manager to work with young adults in its Independent Living Program. This position would work with the Community Housing Assistance Program (CHAP) program, working with DCF involved clients, as well as the Youth in Transition (YIT) program, working with homeless youth who are not DCF involved.

This is a full time position with some schedule flexibility to meet the needs of the clients. This position requires the employee to travel locally in CT to work with clients.

Employees may have access to a shared agency vehicle, but may need to use their own vehicle, as well. Mileage reimbursement is available.

GENERAL STATEMENT OF DUTIES : The Case Manager for CHAP & YIT is responsible for monitoring all aspects of their assigned cases.

Case Managers are involved with case management, life skills instruction, and program management for at-risk adolescents.

ESSENTIAL DUTIES AND RESPONSIBILITIES :

I. Case Management

  • Establish and maintain regular contact with schools or educational program, employer, and significant others involved with the program participants.
  • Meet with assigned participants individually at least on a weekly basis.
  • Oversee the maintenance of assigned cases relative to incident reports, legal documents, case management notes, resident appointments, and any other required information.
  • Work with clients to find appropriate housing that meets program requirements.
  • Attend all pertinent meetings / conferences related to assigned cases.
  • Work closely with the Director in formulating effective, goal-oriented Individual Transitional Living Plans with assigned participants.
  • Act as a team member to cover for other case managers.
  • Use appropriate judgment in connection with appropriate discipline as required, keeping participants in line with program guidelines.
  • Assist clients in daily living skills, as needed.
  • Conduct apartment spot checks.

II. Program Management

  • Assist with intakes and the discharge of participants in the program, which includes data entry of required information for tracking purposes.
  • Provide crisis intervention support as necessary.
  • Transportation of clients, as required. Use of personal vehicle required when agency vehicles unavailable.
  • Prepare reports as indicated in a timely and accurate manner.
  • Ensure program compliance at all times.
  • Participate in team meetings to review each participant and determines appropriate courses of action.

III. Other Responsibilities

  • Provide adequate supervision to ensure participant support and safety.
  • Demonstrate positive rapport with participants, including establishing professional boundaries.
  • Exercise effective group skills and techniques.
  • Other duties as assigned.

Qualifications : Bachelor’s Degree in Human Services or related field, and experience working with adolescents. Must have the ability to set professional boundaries and limits, cultural sensitivity, excellent communication and interpersonal skills, ability to take direction from supervisor, and ability to establish positive rapport with clients.

Requires the ability to organize responsibilities and function independently. Must also have a valid Connecticut driver’s license, a reliable form of transportation and ability to transport clients, and possess the following certifications, or be eligible to obtain these after hire : LIST Training and First Aid / CPR.
