RN Care Manager
Role Summary :
As a Care Manager RN, you will be instrumental in coordinating patient care and managing cases to ensure high-quality, patient-centered outcomes.
You will collaborate with a multidisciplinary team to facilitate effective transitional care and utilization management, addressing the complexities of patient needs and social determinants of health.
Key Responsibilities :
Care Coordination : Collaborate with patients, caregivers, nurses, social workers, physicians, and other healthcare professionals to manage and coordinate patient care across the continuum.
Case Management : Oversee the case management process, including communication, care planning, and transitional care to enhance patient outcomes and ensure efficient utilization of resources.
Patient Advocacy : Support patient self-determination and address various care complexities, including social determinants of health and psychosocial issues.
Other duties as assigned.
Required Qualifications :
Bachelor’s Degree in Nursing or equivalent education / experience.
Master’s Degree in Nursing or a related healthcare field preferred.
California Registered Nurse License upon hire.
Minimum of 2 years of experience in healthcare within Acute, Ambulatory, Post-Acute, ect
Minimum of 2 years of experience in Case Management : Care Coordination or Utilization Management or successful completion of the Transitions in Practice (TIP) program for Care Managers.
Willingness to Travel
