This full-time role offers you the opportunity to make a significant impact by providing vital care coordination and support to individuals in need.
As a Case Manager, you will work with clients to help them navigate complex healthcare systems and connect them with essential services, ensuring they receive the best possible care.
Key Responsibilities :
- Assess client needs and develop personalized care plans
- Coordinate medical, behavioral, and social services for clients
- Act as a liaison between clients, healthcare providers, and community resources
- Advocate for clients to ensure access to necessary services and support
- Monitor client progress and adjust care plans as needed
- Maintain detailed and up-to-date case documentation
- Educate clients and their families on available services and resources
- Collaborate with interdisciplinary teams to deliver comprehensive care
Qualifications :
- Bachelors degree in Social Work, Nursing, or a related field (Masters preferred)
- Prior experience in case management, care coordination, or a related field
- Knowledge of local healthcare systems and resources
- Strong communication and organizational skills
- A client-centered, empathetic approach to care
- Ability to work independently and within a team

Pay Details : $20.00 to $26.00 per hour
