The facility offers 24-hour licensed nursing care, IV antibiotic therapy, pain management, wound care therapy seven days a week, and rehabilitation services, including physical, occupational, and speech therapy.
Baldwin Gardens Nursing Center provides individualized therapy plans to address psychological and biological issues, sustain recovery, and return residents to a normal lifestyle.
Additional services include orthopedic rehabilitation, oxygen therapy and management, nutrition therapy and management, and social services.
The facility accommodates 59 beds.
We are seeking a licensed Speech-Language Pathologist to join our team at a skilled nursing facility. As the Speech-Language Pathologist, you will evaluate and treat residents with communication and swallowing disorders to help them improve their quality of life and functional abilities.
Responsibilities :
Evaluate resident's communication and swallowing abilities using standardized assessments
Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders
Provide skilled speech therapy interventions, including articulation and language therapy, cognitive-communication therapy, and swallowing therapy
Monitor and document resident progress and adjust treatment plans as necessary
Communicate with interdisciplinary team members, including physicians, nurses, occupational therapists, and physical therapists, to coordinate care and ensure optimal outcomes for residents
Educate residents and family members on speech therapy goals, treatment plans, and home exercise programs
Maintain accurate and timely documentation of speech therapy services provided
Participate in team meetings, in-service education, and quality improvement initiatives
Maintain current knowledge of speech therapy practice and relevant regulations
Qualifications :
Current license to practice as a Speech Language Pathologist in the state of employment
Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility
Knowledge of current speech therapy practice standards and regulations
Strong communication and interpersonal skills
Ability to work effectively as part of a team
Ability to manage multiple priorities and meet deadlines
Proficient in using electronic medical records and other healthcare software
