Day in the life of a Hospice Case Manager :

• Care Coordination : Coordinate and oversee the holistic care plan for hospice patients, ensuring it meets their physical, emotional, and spiritual needs.
• Patient Advocacy : Serve as the primary point of contact for patients and families, advocating for their preferences and ensuring their wishes are respected throughout their journey.
• Team Collaboration : Collaborate with interdisciplinary teams including physicians, nurses, social workers, and spiritual counselors to provide comprehensive care.
• Documentation and Compliance : Maintain accurate and up-to-date documentation of patient care plans and interactions, ensuring compliance with regulatory standards and hospice policies
• Current Openings : Full-Time Monday-Friday, Back-up on-call Weekday and weekends
