Job Description

Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

We are seeking an experienced Oncology Clinical Pharmacist to join a hospital team in Dover, Delaware. This is a full-time, days position in an inpatient / outpatient oncology service line.

Schedule : Full Time, Days (80 hours)

Status : Direct Hire

Responsibilities :

Collaborate with oncologists to establish and optimize supportive care measures for patients receiving chemotherapy.

Conduct staff development and educational programs for pharmacy staff, nursing staff, and physicians.

Serve as a subject matter expert in oncology, addressing drug information requests and reviewing care plans.

Round with Oncology Fellows, Nurse Practitioners, and Physicians to optimize therapy per evidence-based guidelines.

Provide patient-specific medication education for oral oncolytics and intravenous chemotherapy.

Review and maintain oncology care plans within the Electronic Health Record for accuracy and appropriateness.

Identify and evaluate medication-related problems using clinical presentation, history, and laboratory data.

Precept pharmacy residents and students.

Participate in Oncology Service Line meetings to improve quality and safety of cancer care.

Promote cost-effective drug therapy and demonstrate focus on cost avoidance and savings.

Required Qualifications :

Doctor of Pharmacy (PharmD).

Active and unrestricted Delaware Pharmacist license (or eligibility to obtain).

pletion of PGY-1 Residency plus at least 3 years of clinical oncology pharmacy practice, OR PGY-2 Residency in Oncology, OR a minimum of 5 years of oncology pharmacy experience.

Board Certified Oncology Pharmacist (BCOP).

Minimum 3 years of hospital pharmacy experience with clinical oncology training.

Preferred Qualifications :

pletion of an ASHP-accredited PGY-2 Oncology Pharmacy Residency.

Experience precepting pharmacy residents or students.
