Personal Care and Support Services (PCSS) Client Care Liaison
Seeking a dynamic experienced healthcare liaison to grow profitable revenues by effectively developing and executing sales and marketing strategies to home care referral sources and prospective clients.
Assists Operations in providing client care coordination and serves as a point of contact for clients, families, and providers to ensure expectations are met or exceeded.
As a PCSS Liaison, here’s a big-picture view of what you’ll do :
- Liaison and coordination with family members, designees, and providers.
- Establish and maintain contacts and relationships with key referral sources for home care including but not limited to : senior residential facilities, long term care insurance, worker’s compensation companies, elder law attorneys, trust officers, financial planners, hospitals, rehabs, hospice and home health agencies.
- Prepares and conducts presentations regarding Interim HealthCare to organizations where prospective clients might be reached or influenced.
- Handle and manage all internal and community based PCSS inquiries.
- Complete initial screening and admissions and perform all PCSS in-home evaluations and paperwork of new clients.
- Complete all admission documentation in a thorough and timely manner, including client’s Electronic Medical Record.
- Identification of resources to address client’s health, daily living, and safety needs.
- Manage Long Term Care Insurance Policies and any applicable authorizations.
- Assist RN Supervisor with Care Coordination, as needed.
- Develop and implement marketing plan for growing corporate contracts, RN Care Management and Medical & Non-Medical PCSS business.
- Assist with conducting patient satisfaction and referral source satisfaction surveys to ensure client and account retention.
- Performing administrative tasks associated with service to clients to include providing information, notices and regular communications with clients and designee.
- Regular telephone contact or home visits per the client’s Service Agreement.
- Completes other assignments as requested and assigned.

A few must-haves for our PCSS Liaison :
- LPN / PTA / COTA / RN degree / experience preferred.
- One year of related business experience or an equivalent of education and work experience may be considered.
- Experience working in the senior field.
- Must possess a valid driver’s license.
- Evidence of essential leadership, communication, education, and counseling skills.
- Proficiency in communication technologies (email, cell phone, etc.)
- Highly organized with ability to keep accurate notes and records.
- Core values consistent with a patient- and family-centered approach to care.
- Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal and nonverbal.
- Demonstrates a positive attitude and respectful, professional customer service.
- Acknowledges patient’s rights on confidentiality issues, maintains patient confidentiality at all times, and follows HIPAA guidelines and regulations.
- Proactively acts as patient advocate, responding with empathy and respect to resolve patient and family concerns, and recognizes opportunities for improvement to meeting patient concerns.
