About The Position

A skilled nursing facility in Oregon City, OR is looking for a dedicated Medical Physical Therapist PT to join their team for a 13-week contract with the possibility to extend.

• xperienced and new grad Physical Therapists PT are encouraged to apply. Responsibilities of a Skilled Nursing Facility SNF Physical Therapist PT :
• Develop effective treatment plans and obtains approval for services from referring physician
• Evaluate residents to obtain data necessary for treatment planning and develop a treatment plan that includes a target list, short-term and long-term goals
• Participate in service training program for other staff in the facility
• Communicate with supervisor and other health team members regarding patient progress, problems and plans
• Possesses and conveys high degree of credibility and integrity
• 13-week assignment
• 36 hours guaranteed
• Submits all billing logs and necessary information on a timely basis Qualifications of a Skilled Nursing Facility SNF Physical Therapist PT :
• Doctorate in Physical Therapy
• Must have an active Physical Therapist PT license
