Haitian Bridge Alliance, Inc. (HBA) also known as The BRIDGE is a 501(c)(3) grassroots nonprofit community organization that advocates for fair and humane immigration policies and provides migrants and immigrants with humanitarian, legal, and social services, with a particular focus on Black migrants, the Haitian community, women and girls, LGBTQIA+ individuals, and survivors of torture and other human rights abuses.
HBA works to elevate the issues unique to Black migrants and build solidarity and collective movement toward policy change.

About the Role
The Haitian Bridge Alliance is hiring 5 case managers to enhance the quality of care and services provided to clients at the Haitian Bridge Community Center in San Diego.
Case managers will provide full cycle case management services which include assessing, monitoring, and evaluating services provided
This is not a remote opportunity . Case managers will work at our community center in San Diego.
Responsibilities
- Assist clients with obtaining and navigating resources
- Under the supervision of the case management director, create case plans, provide housing, medical and educational resources for clients and adjust as needed
- Provide care that is equitable, client-centered,solution focused, safe and in compliance with regulatory standards, Department of Social Services and HIPAA
- Complete intake interviews, input data into software programs, ensure notes are clear and concise
- Follow up with referrals and ensure clients receive necessary services and support in a timely manner
- Collaborate with community agencies, partners and organizations in San Diego
- Complete weekly reporting requirements
- Perform other duties as assigned
Qualifications :
- 1-3 years of experience in nonprofit, social services and / or human services
- 3 years of experience in case management
- Proficient in Haitian Creole and English required, French / Spanish preferred but not required
- Possess or upon employment complete trauma informed care, motivational interviewing, HIPAA, CPR training
- Experience working with San Diego community organizations and agencies
- Possess the ability to use a computer, Microsoft and Google Workspace products and tools
- Possess reliable transportation, valid driver’s license and auto insurance
- Experience with file format, HIPAA, trauma-informed care required
- Ability to handle a fast-paced environment where emergencies often arise; ability to troubleshoot effectively and tackle unexpected challenges
- Experience working collaboratively with people of diverse racial and ethnic backgrounds, socioeconomic status, ages, and sexual orientations
- A strong commitment to social, racial / economic justice,
Annual Salary : $60,000
