Job Description
Peer Counselor will be responsible for assisting in outreach and preparing for and facilitating fitness sessions.
Responsibilities :
- Supporting the participants assigned to them from a particular cohort and program cycle.
- Support with facilitating focus groups to learn more about program success and challenges.
- Documenting and collecting participant responses to be shared with the Data Analysts.
- Helping the Health Coach to deliver the session. This may include preparing and organizing information packages, preparing participation agreement documents, setting up the orientation room, describing their experience in the Program
- Sending text message or calls (as per participation agreement) to schedule the groups.
- Informing the Health Coach if a participant was experiencing transportation and / or other barriers impeding their ability to attend
- Providing Health Coach with feedback from participants about their experience in the program
- Assisting with the collection of attendance and other program activity data from their cohort as directed by the Health Coach
- Assisting Health Coach with distributing participant satisfaction survey and collecting completed surveys
- Attending selected fitness and nutrition classes along with their assigned cohort
Requirements
The ideal candidate will be a graduate of the Healthy Living Program, and has displayed excitement and enthusiasm in continuing their journey through the Healthy Living Program.
The individual must have good oral and written communication skills, be able to follow instructions, and show a high degree of reliability and responsibility.
