About The Position
Job Title : Social Worker Job Specialty : Social Services Job Duration : 13 weeks Shift : Weekends only, Saturday & Sunday, 9 : 00 AM - 5 : 30 PM Guaranteed Hours : No guaranteed hours Experience : Minimum of 3 years as an LISW or MSW in a hospital setting License : Independent Practice Scope Licensure in the State of Practice Certifications : Licensed Independent Social Worker (Clinical), Master of Social Work (MSW) Must-Have : Minimum Master's Degree in Social Work, Marriage and Family Therapy, or Professional Counseling Job Description : Offer brief individual, group, and family therapy, psychoeducation, crisis intervention, as well as providing information and referrals in collaboration with the case management department.
- Review charts in preparation for patient treatment planning and provide input on patient progress.- Evaluate and update treatment interventions in collaboration with the charge nurse and / or Program Director.
- Demonstrate knowledge of the principles of growth and development across the life span, including child, adolescent, adult, and geriatric stages.
- Serve on weekend assessments, group therapy rotation, and clinical-on-call rotation as needed.- Coordinate, develop, and deliver curriculum to meet the psychological needs of patients.
- Participate as a lead in the multidisciplinary treatment team planning process, encouraging patient and family involvement.
- Perform clinical services aimed at stabilizing psychiatric crises and developing healthy mental health management through a biopsychosocial model.
- Responsible for the coordination of discharge plans through discharge planning meetings and follow up with patients, families, physicians, and hospital staff from admission through discharge.
- Initiate patient and family education and discharge planning upon admission.- Arrange for discharge and post-hospital care of patients through institutions and agencies within the community.
- Coordinate the referral of patients and families to appropriate community, social service, home health service, extended care, and rehabilitation facilities, and organizations.
- Create appropriate discharge plans and document information in the medical record.- Assist in identifying opportunities for improvement, participate in data collection and analysis, contribute to problem-solving, and identification of actions to be taken and assess their effectiveness.
- Utilize evidence-based tools to analyze and identify trends / patterns of performance through variance reporting to improve quality and satisfaction and decrease variation and cost.
- Prepare statistical reports on department services and patient needs; participate in billing procedures for services rendered as required.
- Provide education and support to Clinical Staff by attending staff meetings, small groups, and one-on-one sessions related to resource utilization, discharge planning, and psychosocial aspects of healthcare delivery.
- Facilitate group psychotherapy and psychoeducational focus groups serving as a role model for patients to observe and learn socially effective values, attitudes, and behaviors.
- Document patient care and information in an accurate, legible manner using correct grammar and spelling; documentation should reflect patient behavior, thoughts, feelings, and responses to milieu and treatment plan-directed interventions.
- Maintain professional, assertive, and respectful interactions with others while demonstrating a caring attitude and behavior.
- Possess knowledge of payor practices, regulations, standards, and reimbursement related to government and non-government entities.
- Have a background in and knowledge of hospital-based and / or third-party payor case management, utilization management, and discharge planning.
- Document in accordance with CMS and The Joint Commission standards.- Promote a positive team atmosphere and develop collegial working relationships within the hospital environment, participating on committees as assigned.
