Primary Purpose :
The person in this position is a member of the H4 Medical Respite team where each team member plays an integral role in providing care to Respite patients.
S / he will provide human compassion and empathy with the nursing process of assessment, planning, implementation, and evaluation to provide H4's chronically homeless and underserved patients with dignity and quality care.
S / he is expected to be an active participant of the interdisciplinary team, advocates for, and leads the coordination of the medical and social needs of H4 patients.
Essential Duties :
- Conduct physical exams to assess patients' health needs and problems
- Implement providers' orders, perform treatments including, but not limited to, starting IVs, blood draws, wound care, etc.
- Administer medications
- Coordinate care among PCPs, specialists, behavioral health, other healthcare organizations, etc
- Review and maintain medical records
- Implement and develop nursing care plans
- Counsel patients on disease prevention and health maintenance
- Educate patients about their medical conditions and treatment plans
- Provide emotional and psychological support
- Supervise nurse aides and / or medical assistants
- Maintain a safe and hygienic working environment
- Prepare exam rooms, medical equipment, and decontaminate instruments
- Assist H4 Clinical Director with quality audits and ensure that the organization complies with Federal, State, and local guidelines and standards
- Assist Clinical Director in coordinating new hire orientation and educational in-services for the clinical staff
- Other duties as assigned
Regular and reliable on-site attendance is required.
Other Duties :
- Perform other related duties as assigned
- Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions;
use equipment and materials properly.
Minimum Qualification Requirements :
Education :
- Bachelor's Degree or higher in Nursing preferred
- Minimum education of Associates Degree in Nursing accepted
- Current unexpired State of Hawaii RN license required
- BLS / CPR Certification required
Experience :
Two years of clinical experience preferred
Skills / Knowledge :
- Clinical experience working with communities and providing community health education outreach preferred.
- Comfortable working in managed care / multi-professional, multi-cultural environment
- Ability to assess and plan in a multi-cultural context.
- Professionalism, sound clinical judgment and skills and emotional stability
Work Hours : Shifts from 7am-7pm or 7pm-7am at 3 times per week and every third weekrequires 4 working days to get an average of 40 hours per week over a 3 week cycle period (36 hours, 36 hours, 48 hours 40 hours average).
Must be willing to work the occasional holidays, evenings / nights, and weekends.
Hours and days may vary based on the needs of the organization
