Responsibilities
The Quality Improvement / Staff Education Nurse (RN) will :
- Participate in the development, implementation and evaluation of training, orientation and, in-service programs and competency assessment for all Home Care staff
- Provide support of all clinical staff in the proper use of the Horizon Home Care clinical application
- Conduct clinical record reviews and develops and implements educational programs in areas identified as requiring additional education to promote superior patient outcomes.
- Plan, implement and evaluate the effectiveness of the clinical documentation system and works with the IT staff to modify the system to meet changes in documentation requirements
- Participate in the agencies QAPI Program
- Perform other duties, as needed
Qualifications :
- 3+ years of Home Care experience
- Bachelor's Degree in Nursing
- NYS Registered Nurse (RN) license
- Experience in Quality and / or Performance Improvement
- Experience with Staff Education
- Informatics experience
Desired Skills :
- Master's Degree in Nursing
