Overview :

Altus Hospice is currently seeking a Full-time On-Call Registered Nurse (RN) to join our team, providing exceptional care and support to hospice patients and their families during their most critical moments.

As an On-Call RN, you will play a vital role in identifying patient and family needs, coordinating care, and offering preventative measures and compassionate support.

Responsibilities :

Be the Lifeline : Respond to a variety of patient needs, from urgent to routine, your unwavering support will make a difference in critical moments.

Provide Empathetic and Heartfelt Assessments : Conduct comprehensive assessments of patient's physical, emotional, social, and spiritual needs.

Maintain Accurate Documentation : Every detail is crucial in delivering high-quality care, both in the patient's home and within our agency and regulatory standards.

Qualifications :

Bachelor's degree in nursing or health care preferred.

Hold current unencumbered license as a Registered Nurse (RN).

Minimum one (1) year experience as an RN in a medical surgical / acute care setting. Hospice exp a Plus!

Must be computer proficient in typing and various programs, including background in EMR.

Possess and maintains current CPR certification if required by state.
