Extremely fun and interesting Hormones and Wellness Practice is seeking a bright, fun and energetic person to join our team as a Part-Time Health Coach.
This position will be the main contact point for patients in our weight loss program and will be responsible to meet with them on a weekly basis.
The Health Coach will work closely with patients, providing guidance, support, and motivation to help them achieve their weight loss goals.
This program does include the use of prescribed medications as deemed necessary by one of our providers.
During the weekly check-ins the Health Coach will be monitoring patient progress, managing side effects, guiding titration of the medications, and offering broad recommendations following our Standard Operating Procedures established by our licensed medical professionals.
The practice has a specific culture and a dedication to quality care, patient satisfaction, teamwork and creating a fun enjoyable work environment is crucial.
Currently this is a Part-Time position as our weight loss program is in its early stages. As this program grows this position can grow with it as well providing more hours per week.
General Responsibilities
- Conduct weekly meetings with patients, either in-person or via video conference, to review progress and provide support.
- Monitor and document patient progress, including weight loss, adherence to the program, and any reported side effects from medications.
- Guide patients through medication titration schedules following the clinic’s SOPs.
- Serve as the main point of contact for patients regarding any concerns or questions related to their weight loss journey.
- Collaborate with the clinical team to ensure each patient receives personalized care and attention.
- Maintain accurate and up-to-date patient records in the clinic’s EMR (Cerbo).
- Provide patients with educational resources and lifestyle recommendations to support their weight loss goals.
- Participate in regular team meetings to discuss patient progress and address any challenges.
Qualifications :
- Certification in health coaching, nutrition, or a related field.
- Experience in a clinical setting, preferably in weight management or chronic disease management.
- Strong understanding of nutrition, weight loss strategies, and behavior change principles.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Familiarity with telehealth platforms and electronic medical records (EMR) systems.
