POSITION
Providing IN HOME Services for an Adult with Developmental Disabilities Support, Inc. is looking for an EXPERIENCED Host Home Provider to contract with our company! If you live in Lakewood, Colorado, and your home is wheelchair accessible, you now have the opportunity to open your OWN home to provide for someone with developmental disabilities.
You will be contracting with our company (Support Inc.) and will be able to help someone’s quality of life. You can help someone who has significant medical concerns or behavioral challenges and make a difference.

SPECIFIC CLIENT INFORMATION / NEED :
- Client needs a fully wheelchair accessible home, for an electric wheelchair, preferably with a walk-in / roll-in shower - or a bathroom that’s large enough for lift access.
- Client will require assistance with transfers, bathing, toileting, and meal preparation.
- Client will have a window of unsupervised time at home during the day, and works one day a week at a location in Lakewood.
- Client has a strong desire to maintain independence and to stay connected with family who would offer occasional weekend respite stays.
Provider Duties and Responsibilities
Provide personal care
Assist with hygiene care, meal preparation, dressing
Provide a variety of recreational activities to enhance social development
Include client in family life and activities
Accompany client to supermarket, museums, parks, dances, recreation centers, etc.
Provide transportation as needed
Assist with medical care, including appointments and administering medications
Provide documentation of progress notes and medical records
Provide training on life learning skills
To be successful, candidates . . .
High school or equivalent, minimal
Must complete and pass criminal and motor vehicle background checks
Must complete training for Medication Administration, CPR / First Aid and maintain ongoing training requirements
Must have at least 1 year of experience as a host home provider
Wheelchair accessible home, required for this specific position
Experience with behavioral needs, preferred
Experience with medical needs, preferred (i.e. CNAs, LPNs, RNs, etc.)

Job Type : Contract
Salary Range : $18,000.00 to $65,000.00 / year depending on the clients’ disability level
