About The Position
Job Title : Registered Nurse Job Specialty : Wound Care Job Duration : 6 months Shift : Day - 8x5 - 8 AM to 4 PM Guaranteed Hours : 40 hours per week Experience : Minimum 2 years of experience required License : IN RN License Certifications : WOCN, CWS, or WCC;
BLS certification through the American Heart Association required Must-Have : - 3 years of recent Wound Care Coordinator experience- IN RN license attached to resume- Certifications attached to submission Job Description : Manage the facility's wound care program, including standards of care and practices related to wound, ostomy, and continence patient care needs.
- Provide direct patient care and assistance to staff nurses and act as a consultative service to affiliated healthcare agencies.
- Develop and implement the facility wound care program in conjunction with the national standardization process, including patient care protocols, documentation tools, wound care formulary, and referral criteria.
- Establish standards of care, competencies, policies, and procedures in quality, cost-efficient, and effective wound care for all clinical staff.
- Act as the primary consultant to Wound Care Clinicians and a consultant to facility staff, advocating with physicians; enable clinical staff to effectively assess wounds, recommend appropriate protocols, and initiate plans of care through training and support.
- Provide ongoing education to staff on products available for use in the hospital.- Evaluate all wounds upon admission and ongoing to determine treatment plans and provide early problem identification.
- Provide consultation and / or assessment on patients with pressure injuries, consulting on wounds that do not show measurable signs of healing within two weeks.
- In consultation with the physician, assist the primary nurse in developing an appropriate plan of care for comprehensive wound management and prevention.
- Make recommendations to the physician for changes to wound care orders and provide evidence-based research support as needed.
- Review medical records of patients with wounds to ensure wound assessments, care plans, and treatments are clearly and correctly documented and that appropriate wound-related treatments are being provided.
- Investigate all cases with adverse events related to wounds by completing root cause analysis and developing action plans based on findings with nursing leadership.
- Participate in clinical outcome monitoring, follow-up, and agency performance improvement initiatives.- Participate in quality data reporting through wound-related data set records completion.
- Collaborate with support center clinical operations staff to maintain a cost-effective wound care formulary.- Assist as needed with training of new hire Wound Care Coordinators within the hospital division.
