Only QUALIFIED Healthcare Professionals accepted) Psychiatry / Mental Health-NP - A non-profit health and human services organization providing a variety of programs, services, and resources to very low and low to moderate income, Asian, Pacific Islander, Latino, Middle Eastern, East African, African American and other diverse ethnic populations of San Diego County is seeking a psychiatric nurse practitioner to join their behavioral healthcare team.

The Treatment and Counseling Center provides outpatient mental health treatment for adults and older adults with serious mental illness and promotes psychosocial rehabilitation principles of wellness and recovery.

The Centers target population is Pacific Islander adults, ages 25 and older and Pacific Islander Transition Aged Youth ages 18-24.

Under the direction of the Psychiatrist and Program Manager, the Nurse Practitioner provides treatment to clients with all ranges of psychiatric symptom acuity and possible co-occurring substance abuse.

The Psychiatric Nurse Practitioner Conducts initial psychiatric evaluations for clients that have been referred to and accepted into the program.

Provides diagnose and treatment of clients. Evaluates and prescribes medication as indicated. Orders labs and reviews results with clients and staff as needed.

Functions as a member of the treatment team and provides input in client treatment plans. Prepares accurate timely medical records of all services rendered including Psychiatric and medical history and medication follow up.

Provides coordination of care with outside treatment providers, including primary care services. Communicates and collaborates with pharmacies for timely and accurate refill of prescriptions.

Works in conjunction with the Psychiatrist to develop the most appropriate medication regimen given the clients history and presenting symptoms.

Works closely with the client and program staff to help enhance medication adherence through client, family, and staff education.

Performs crisis intervention and medication management. Monitors and ensures compliance with medication procedures at program sites.

Facilitates client linkage to community-based primary care providers. Provides staff training on various topics related to the provision of Medical and Psychiatric services to target population.

Participates in utilization review and treatment team meetings as scheduled. Attends staff meetings, trainings, and other required meetings.

Preferred Qualifications : Bilingual skills in Asian Pacific Islanders languages (Vietnamese, Cambodian, Lao, Chinese, etc.

Demonstrated knowledge of County HHSA BHS requirements, including documentation. Knowledge of Cerner (aka Anasazi), electronic health record systems.
