Hourly Pay Range :

$35.16 - $54.50 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

• Hours : 8a-6 : 300, 10 : 00a-8 : 30p

A Brief Overview :

Assesses the clinical needs of customers seeking behavioral health services and provides treatment referrals which are consistent with the customer's medical and psychiatric status / symptoms at Edward Hospital and Elmhurst Hospital. Assists in coordination of discharge planning services to patients who need ongoing behavioral health care following discharge. In collaboration with other members of the healthcare team, assesses behavioral health needs of the patient and offers suggestions regarding follow-up care options. Prepares and updates medical records including nature of discharge needs, resources available, patient and family response to recommendations and the final discharge plans arranged. Coordinates with or directs patients to designated community agencies or resources, as necessary. Mentors new BH Crisis Case Manager to the department. Leads departmental work groups in performance improvement activities or care conferences and huddles.
