Job Summary :
Under supervision, carries out a variety of treatments, procedures and technical tasks to assist in the nursing care of patients.
Serves as an active member of the multi-disciplinary patient care team in the practice. Provides for the physical, mental, emotional safety and hygienic needs of patients and assists in assuring effective and efficient coordination of nursing care activities within the practice.
Processes forms, records, and other materials, transcribes insurance information and medical records from forms into electronic medical systems, checks reports and patient’s records for accuracy, compiles information for special studies and interacts with customers.
Explains policies / procedures, obtains and routes patient information. Assesses nature of phone calls, and triages as necessary.
Arranges transportation to hospital as necessary. Assists in the office as needed. Practices excellent communication skills with all patients, including those from vulnerable populations.
Performs office and administrative support duties to assist in the effective operation of the clinical site.
Communicates with Answering Service / other Practices as directed and per policy.
Ensures that all medical records / reports are reviewed and processed in electronic medical record system.
Able to skillfully operate various office equipment including fax machine, copier, credit card scanners and other equipment specific to the location.
Assists Office Manager and staff with any projects or requests.
Handles incoming calls, routing as necessary.
Maintains work space in a neat, clean order.
Assists with Care Coordination by closing identified gaps in care for the practice’s population
Ensures Specialist Authorization and Certifications Are Completed For All Visits
Responsible for obtaining all authorizations for the specialist according to policy
Accurately documents all authorization data in system
Identifies carved out insurance benefits on a case by case basis and documents accordingly
Effectively provides patient information to insurance companies to secure medical necessity for treatment plan documented by the provider
Collects Patient Data and Materials When Working in Clinical Capacity
Obtains health history and base line patient data from patients
Performs routine physical assessment functions such as blood pressure, temperature, pulse, visual test, EKG, Pulmonary Function Test
Collects urine and blood samples in accordance with established protocols
Uses discretion in disseminating patient information to outside sources
Performs simple lab tests (CBC, glucose, HKL, etc.) per policy and procedure
Reviews patient record for completeness prior to visit
Provides entries to patient records as necessary in preparation for provider review
Monitors the return of lab / test results. Assures all results requested are complete and available for provider review
Assists With Patient Care and Maintains Clinical Areas
Delivers on-site care for patients under Medical Provider direction
Assists in the delivery of patient care such as suturing, wound care. Debridement, etc.
Assures callbacks to patients under medical direction are in accordance with protocol
Responsible for restocking exam rooms as necessary with instruments and medical supplies
Responsible for performing injections under Medical Provider direction and for maintaining proper inventory of vaccines and other injectables
Cleans and prepares exam rooms per protocol; sterilizes instruments as necessary
Monitors and arranges for proper disposal of lab and medical waste / supplies
Performs quality controls and maintains records for lab equipment / refrigerators
Performs medication authorizations
Refills prescriptions under authorization of provider
Supports patients, families and caregivers in self-management, self-efficacy, and behavior change

Position is 20 hours per week. Hours are first shift matching the operational needs of the practice.
EDUCATION / MINIMUM REQUIREMENTS :
Graduate from an accredited school of nursing.
STATE / FEDERAL MANDATED LICENSURE OR CERTIFICATION REQUIREMENTS :
Current State of Connecticut license as a Licensed Practical Nurse.
PHYSICAL REQUIREMENTS :
Must have the ability to perform all aspects of nursing care. Must be able to lift a minimum of 50lbs. Must be able to endure a prolonged period of extensive walking, standing, stooping, pushing, and pulling.
Moderate periods of sitting, twisting, reaching, occasional crouching, crawling, kneeling, and balancing. Vision, hearing, smell and tactile sensation corrected or accommodated to normal.
WORK ENVIRONMENT :
Regular contact with residents, body substances, and hazardous chemicals, 75 degree temperature, varied lighting, regular contact with soap and water, moderate noise level, use of various types of lifting devices.
COGNITIVE REQUIREMENTS :
Ability to read, write, and follow written and oral instructions.
