As a Licensed Practical Nurse (LPN) in Home Health Care, you will :
- Provide professional nursing services under the direction of an RN in compliance with state regulations and company policies.
- Offer skilled nursing interventions for patients' illnesses, rehabilitative needs, and preventative care.
- Confer with the RN Supervisor on necessary changes in the Plan of Treatment and accept verbal orders from physicians.
- Maintain the highest standards of professional conduct and confidentiality in patient care.
- Maintain contact with patients, physicians, and other healthcare team members in a timely manner regarding patient needs and status changes.
- Prepare, submit, and maintain documentation as required by the Company and / or facility.
Skills and Experience Required :
- Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years experience as an LPN / LVN in a clinical setting, preferably in home health or hospice.
