Position Summary :
As a Care Coordinator, you will provide a variety of direct nursing services to help patients maintain the highest level of care and independence.
We seek a dynamic, customer-oriented individual to serve our clients and care staff, assisting with ongoing case management and staffing support.
In this role, you will match our clients and caregivers and act as a case manager for the ongoing needs of our clients, helping them live assured with Griswold.
Key Responsibilities :
- Ensure clients receive the best-matched care by assigning appropriate care staff to cases
- Maintain communication and strong relationships with clients and caregivers
- Conduct retention visits and introductions for clients and caregivers
- Perform thorough assessments to understand clients’ objectives, challenges, and care requirements
- Participate in on-call responsibilities on a rotating basis
- Adhere to all company policies and procedures
Requirements :
- Relevant experience in a related role; specific experience in home care and scheduling is a plus
- Strong verbal, written, and interpersonal skills
- Excellent computer skills, proficient with Microsoft Office
- Pleasant phone manner
- Strong desire to help others
- Creative problem-solving skills, organized, and detail-oriented
LPN Duties :
- Perform assessments to establish services and care plans for clients
- Conduct quarterly assessments
- Supervise aides and conduct observations
- Conduct training and staff development programs
- Review care notes and care plans for accuracy or changes in client status
- Respond to after-hour clinical calls and emergencies (minimal)
