Job Description :
- Utilize the nursing process to carry out comprehensive assessments of patient / caregiver needs to provide quality care, in accordance with State and Federal regulations and policies.
- Develop and communicate a realistic plan of care for each patient, ensuring the inclusion of patient / caregivers in the process.
- Participate actively in interdisciplinary team (IDT) meetings to develop individualized care plans within five days of admission, and review every fifteen days.
- Serve as Case Manager for assigned patients and manage caseloads and visit times in line with the IDT plan of care.
- Implement medical care plans, nursing interventions, and procedures to ensure safe and effective patient care.
- Stay up-to-date on pain control, symptom management, high-tech skills; including IV use, wound VAC, complex wound care, etc.

- Delegate duties to LPNs, review documentation, annotate visit notes, and act on recommendations to meet patient needs and ensure the care plan is followed.
- Evaluate various aspects of the implemented care plan, including identified problems and patient responses, to ensure overall quality of care.
- Maintain accurate and up-to-date documentation of nursing care provided for effective integration of information among the healthcare team.
- Follow agency protocols with McKesson processes, including locking down visits and completing follow-up notes.
- Educate patients and caregivers on informed decision-making, including medication side effects, treatment options, and discharge instructions.
- Accurately complete various forms, such as Medicare Notice of Non-Coverage, in a timely manner during visits.
- Conduct supervisory visits according to regulations and communicate relevant information to the Scheduler.
- Assess patients' needs for hospice aide services, develop and coordinate plans of care, and provide ongoing supervision.
- Report patient incidents (infections, injuries, etc.) to the Quality Improvement Specialist promptly.
- Coordinate safe patient discharges and / or transfers and ensure all paperwork and patient information are appropriately processed.
- Coordinate patient care with physicians, providers, team members, and vendors to support the recommended plan of care.
- Participate in monthly peer review audits and provide / accept constructive feedback professionally.
- Obtain prior authorization for services when necessary and engage in ongoing communication with insurance case managers regarding services rendered.
- Develop and conduct educational presentations for various facilities and community agencies.
- Facilitate patient transfers efficiently and complete necessary documentation.
- Participate in bi-weekly team meetings to ensure effective communication and maintain documentation.
- Advocate for patient care issues to ensure the quality and sensitivity of care to each patient's needs.
