Requirements :
- Active unrestricted license to practice medicine in any State, Territory, or Commonwealth of the United States
- Board Certified / Board Eligible by the American Board of Internal Medicine in Pulmonary Disease
- BLS (AHA)
- ACLS (AHA)
- DEA Certificate
- NPI
