Salary Compensation Range : $42,000-46,000
MAJOR FUNCTIONS :
The Housing Case Manager is responsible for substance abuse counseling and intervention services. The Housing Case Manager must be able to stabilize and promote self-sufficiency and optimum health for people living with HIV / AIDS, Mental illness and / or substance use disorders.
Manage a case load of 20 to 25.
The Housing Case Manager provides advocacy as well as coordinate services through home visits.
SPECIFIC DUTIES & RESPONSIBILITIES :
- Provide substance abuse intervention, counseling, educational services, and case management services to tenants.
- Evaluate and recommend treatment plans for tenants with mental health and chemical dependency problems after a one-to-one interview.
- Provide regular, individual, and group counseling services.
- Establish community and agency affiliations to strengthen the delivery of services for tenants, including medical & detoxification when necessary.
- Attend case review conferences with HRA / HASA and provide written evaluations regarding tenant’s drug problems and money management issues.
- Present case load weekly at chart review.
- Meet for supervision with Assistant Director bi-weekly.
- Orient tenant’s to the program upon admission.
- Maintain up-to-date tenant’s charts, case notes, service plans and psycho social.
- Attends all in-service training.
- Responsible for collection of tenants monthly rent.
- Set a relaxed tone and social environment (by role modeling)
- Other duties as assigned
REQUIREMENTS : EDUCATION AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES :
- High School Diploma, one (1) to two (2) years of experience in counseling or
- Two (2) years of Odyssey House experience and a High School Diploma, or Bachelor’s Degree in Human Service.
- Knowledge of mental illness, substance use, HIV / AIDS, and homelessness.
